Description
Provides education and services for the prevention, management and treatment of diabetes
  • Provides case management and screening services
  • Offers basic foot care clinics
  • Offers nutrition programs and recipe exchanges
  • Works to reduce the number of avoidable complications that can arise from uncontrolled diabetes
  • Advises on developing and implementing an approach to care and management of diabetes
  • Assists individuals in finding appropriate services
  • Provides referrals to other health care professionals and the Northern Diabetes Health Network (NDHN)

  • Breakfast/lunch Program
  • Organized Walks
  • Fitness and Nutrition Programs
  • School Diabetes Programs
  • Support group
  • Traditional herb/medicine workshops
