Description
Staffed with registered nurses who have specialized training in the monitoring of blood work (INR - International Normalized Ratio testing applies to patients who take Warfarin)

Program may offer the following:
  • Medication Adjustments
  • Assess for minor and major bleeding episodes
  • Provide education sessions
  • Assist patients with concerns with their anticoagulant therapy
  • Follow up with patients that miss lab visits
  • Follow-up with pharmacy to provide Warfarin refills
  • Coordinate anticoagulant bridging for patients requiring time off of Warfarin (for surgery, etc)
  • Referrals to other programs or agencies
Hours of Operation
Mon-Fri 8 am-12 noon, 1 pm-4 pm

Individuals who require specialized blood work monitoring
