Transition House
- Description
- Operates a 10-bed rehabilitation housing program for individuals struggling with severe and persistent mental health issues
- Assists adults with severe mental illness achieve an optimal level of functioning so they can return to community living
- Provides three Homes for Special Care (HSC) beds which serves as permanent housing
- Assists individuals in working towards the goal of transitioning into the community within two years
- Assistance with daily living activities
- Medication management
- Ensuring individuals attend appointments and receive therapeutic services, such as counselling
- Eligibility
- Individuals 16 years of age and older struggling with severe and persistent mental health issues who have frequent psychiatric hospital admissions and numerous hospital days requiring medium to low support
- Must have a psychiatric diagnosis and be under the care of a Physician and treatment team
- Must have the capacity to adapt to living cooperatively with others
- Cannot be a threat to the general safety of others
- Must have control of substance abuse if applicable
