  • Provides intensive treatment, rehabilitation and support for individuals struggling with severe and persistent mental health issues
  • Offers advocacy including attending necessary appointments, assistance in getting services such as housing, court services and medical treatment
Support services include:
  • Assertive outreach
  • 24-hour crisis assessment
  • Supportive counseling
  • Medication monitoring
  • Skills building
Eligibility
Individuals 18 years of age and older who have complex bio-psycho-social needs due to severe and persistent mental illness such as Bipolar disorder, Schizophrenia and Schizoaffective disorder
