Description
  • Provides health care services for ill, injured or elderly community members to allow them to remain in their own homes
  • Provides services through Registered Nurses, Registered Practising Nurses and Personal Support workers

Provides the following services:
  • Assessment to determine individuals health history and develop a personalized care plan
  • Case management to ensure that care plans are being carried out and that the plan developed is still suitable
  • Emergency response (Life line)
  • Home care assistance
  • Personal care help with activities such as dressing and bathing
  • Basic foot care
  • Teaching the individual in need to care for themselves
  • Friendly visiting and security checks
  • Home support with light housekeeping, laundry and meal preparation
  • Respite for family members and caregivers
  • Home Visiting Nurse for wound care and chronic condition monitoring
  • Meals on Wheels
  • Occupational Therapy
  • Transportation
