Transitional Youth Program

  • Provides support to youth with developmental disabilities to create a life plan for the transition to life after graduating from high school
  • Assists with various applications such as housing, passport, special services and Ontario Disability Support Pension
  • Advocates on behalf of the family and individual

Offers guidance and support for:
  • Community resources
  • Education and learning opportunities
  • Employment and training opportunities
  • Recreation and leisure
  • Volunteer experiences
Hours of Operation
Mon-Fri 8:30 am-12 noon, 1 pm-4:30 pm
First Nation (FN) individuals living in any of the FN communities in the Sioux Lookout district
Individual must:
  • Agree to program by choice
  • Have an identified developmental disability
  • Be leaving school or have been out no more than 2 years
  • Not be receiving accommodation support by the Ministry of Children, Community and Social Services (MCCSS)
  • Have little or no daytime activities appropriate for their strength and need
  • Agree to participate in a planning process, along with family and interested persons, to identify outcomes to be achieved and supported

Age Requirements: Ages: 16 year(s) - 24 year(s);

Residency Requirements: The 33 First Nation communities in the Sioux Lookout region

Intake assessment required with Transition Facilitator
Fully Accessible ; Partially Accessible - Parking -- Accessible with one designated handicapped parking space; Entrance -- Accessible with ramp but no automatic push button doors; Washroom -- Accessible with side holding bar
