Description
  • Functions as a short-term inpatient assessment, treatment, and rehabilitation program for adults with a moderate to profound developmental disability and mental health concern
  • Offers a short-term stay, up to 18 months
  • Helps individuals keep and return to their homes
  • Provides care plans that are based on an individuals interest and ability level

Office hours: Mon-Fri 8:30 am-4:30 pmMon-Sun 24 hours
Eligibility
  • Adults with severe and persistent mental illness who have not responded to treatment and/or have exhausted all district level supports and interventions
  • Individuals must be medically stable
  • Individuals must not have any current criminal charges or active legal restrictions
  • Must have a signed letter of commitment from the referring agency or service
