Description
Provides culturally appropriate case management and discharge planning to Aboriginal patients

Services include:
In hospital
  • Cultural support and advocacy
  • Discharge planning and resources
  • Providing cultural resources in community
  • Staff cultural awareness and education
  • Family supervised visits to community and home

Community services
  • Capacity building for patient's returning home
  • Education and support
  • Clinical Consultation and Peer support
  • Family support and advocacy
  • Patient follow up and support in community
  • Involvement in community and professional event planning
Hours of Operation
Mon-Fri 8:30 am-4:30 pm
Eligibility
Aboriginal individuals who are going to be discharged into their home communities

Age Requirements: Ages: 16 year(s) and up;
Application Process
Individuals are referred through the North Bay Regional Health Centre
