Description
  • Designed to assist individuals with a history of serious and persistent mental illness to secure and maintain permanent, safe, and affordable housing
  • Helps individuals find secure housing when they are making the transition from hospital to home, or making changes in the community
  • Teaches individuals the life skills needed to maintain a home
  • Assists individuals in dealing with landlord and legal issues
  • Offers the Homelessness Initiative that provides rent supplements to those who meet eligibility criteria

Diagnosed with a serious mental illness (such as schizophrenia or bipolar disorder)
